All you have to do is download the Registry App, complete the registration process, and fill out all the Medical Surveys in the Activities section. The winner will be notified by email at the end of PPMD’s Virtual Annual Conference.

Already a member of The Duchenne Registry? No worries – you’re automatically entered if you have completed all of these steps prior to our Virtual Annual Conference! 

To be entered into the Smartphone Raffle, please complete all 3 steps below:

1. Download The Duchenne Registry App**
2. Complete the Consent and Registration process
3. Fill out all Medical Surveys in the Activities section

• The smartphone raffle is for US registrants only.
• Multiple entries are not allowed – each affected/carrier individual can only have one Registry account.

**If you are brand new to the Registry, the best place to start the process of downloading the Registry App is our website, DuchenneRegistry.org. Click JOIN and follow the instructions. You can also read the App Q&A section where you will find helpful how-to videos. If you previously participated in the web-version of our Registry, please use the invitation email with the activation code sent to you last November when the App launched. If you cannot find that email, please contact us to have it resent to you.
