  • MinnesotaCare Children’s Health Insurance Program
    This program provides health insurance to eligible children under age 21 who have no health insurance coverage.  Financial eligibility and monthly premium based on family income and size.

  • Medical Assistance TEFRA for children with disabilities
    This program provides health insurance to eligible children with a disability as defined by the Social Security Administration and receive a disability determination by the State Medical Review Team.  Financial eligibility is based on the child’s income, yet some families are required to pay a monthly “parental fee”.

  • Community Alternative for Disabled Individuals (CADI) Waiver
    This Home and Community Based Services Waiver provides supports and services to children and adults with a disability who are eligible for the Medical Assistance program and meet Nursing Facility Level of Care criteria allowing them to remain at home and in the community.  Services include home accessibility modifications, home health aide and nursing, personal care assistance, specialized supplies and equipment and consumer directed supports.
