Health & Well-Being

  • Children’s Health Insurance Program (CHIP)
    CHIP provides health insurance to eligible children under age 19 who do not have health insurance.   Financial eligibility is based on family income and size.

  • Elderly and Disabled Waiver
    This Home and Community Based waiver program provides services to people who meet Nursing Facility Level of Care criteria and are financially eligible allowing them to remain at home and in the community.  Services include home health visits, personal care and respite services.

  • Independent Living Waiver
    This Home and Community Based waiver provides services to eligible children age 16 and/or older with severe orthopedic and neurological impairments who meet Medicaid financial criteria.  Services include case management, personal care attendant home accessibility modifications and specialized medical equipment and supplies.
